Student Health Record Requirements
All full-time students are required to submit a completed health form, referred to as the Student Health Record, that includes a medical history, recent physical exam and documentation of the required immunizations and testing. This Student Health Record is also available at the Student Health Center, and must be submitted prior to attending classes by new students.
The University reserves the right to deny housing, to institute fines or to withhold registration for any student who does not meet the pre-matriculation requirements established by Student Health Services.
Can I get my physical exam at the Student Health Center?
No, the physical exam must be completed prior to your arrival to the University.
Where can I get a physical exam?
You may obtain a physical exam by:
- An outside Healthcare provider in private practice
- An outside convenience clinic (CVS MinuteClinic, Rite Aid RediClinic, Walgreens Take Care Health, Drexel Convenience Care Clinic, Walmart, etc.)
- A local public health department
