Transcript Request Information

An official transcript is placed in a sealed envelope and bears the University seal and signature of the University Registrar. Official copies of transcripts will be sent to other colleges and graduate schools upon request by the student to the University Registrar's Office. 

In addition, unofficial copies of the transcript are available to the student. 

As a service to the student, copies of the transcript are issued free of charge.

In order to release a transcript, the University Registrar's Office needs to verify the identity of the student as well as receive a formal, written request bearing the signature of the student.  To request a transcript, please do one of the following options:

  1. Download Transcript Request form, complete and send to the Registrar's Office. 

  2. Visit the University Registrar's Officeon the first floor of Archer Hall to fill out a Transcript Request form. 

  3. Put your request in writing to the University Registrar's Office.   Please include your name, student number, approximately the last semester you attended, the full address of where you would like it sent, state whether it should be an official or unofficial copy, and your signature.

Change of Name, Address, or Student Number Information


It is the student's responsibility to see that a valid permanent address is on file in the University Registrar's Office.  Any change of permanent or local address must be reported to the Registrar's Office when it occurs.  A forwarding address should also be given to the U.S. Postal Service.

It is also the student's responsibility to inform the University Registrar's Office of a change in name or student number. 

All changes of names, addresses, or student numbers need to be made in writing.  Students can do one of the following to update this information:

  1. Down load the Change of Address Form, complete and send it to the University Registrar's Office.

  2. Visit the University Registrar's Office on the first floor of Arhcer Hall to fill out a Change of Name, Address or Student Number form.

  3. Put your request in writing, including your old and new information and your signature. 

NOTE: ANY CHANGE IN STUDENT NUMBER MUST BE SUPPORTED BY A COPY OF THE STUDENT'S SOCIAL SECURITY CARD.

Student Enrollment Certification

Students in need of official certification of their enrollment may request this certification using one of the following options:

  1. Down load the Enrollment Certification form, complete and send it to the University Registrar's Office.
  2. Visit the University Registrar's Office on the first floor of Archer Hall to fill out a Enrollment Certification form.
  3. Put your request in writing, including your name, student number, where you would like the certification sent and your signature.
